In the event a Nex-Tech Wireless customer would like to separate line(s) that are currently active on a shared account, a form is available that requires authorization from the current account holder to move the service(s) as well as acceptance from the new account holder assuming financial responsibility for the service(s).
Signature is not required at the same time, but the form will need to be signed by both parties witnessed by an NTW employee or a notary. An account will need to be established for the new account holder, if one does not exist. New Account creation is subject to credit check and a copy of Photo ID required.

The Safe Connections Act, (SCA) has been enacted by Congress to ensure survivors of domestic and other related crimes and abuse violence can separate from abusers without losing independent access to their mobile service plan. Mobile service providers must separate the line of service of a survivor and any individual in their care from a mobile service contract shared with an abuser.
- The survivor is not required to know or provide a password to request moving their line off an abuser’s account. If the customer wishes to port out their line to another carrier, they can do so without penalty. They will need to have their new provider reach out to Nex-Tech Wireless (NTW) to request the port out WITHOUT a Port PIN.
- The SCA form to move service(s) can be completed by the survivor to start the process and is linked below. A printed copy can be provided at any NTW location.
- Assigned rate plan and features can remain on the line or currently offered plan can be selected, for plan information, see https://www.nex-techwireless.com/plans.
The completed form can be submitted by email to NTW at officeofprivacy@ntwls.com along with all other necessary documents (delivery email is also included on the form). - The request and any information submitted will be handled with extreme confidentiality.
- A member of the NTW internal team handling the request will be in contact within 2 business days.
- The documentation and information will only be used to implement the request, provide service, and manage the survivor’s account.
- If a new account is created, information will be collected to perform a credit check but the results of a credit inquiry will not result in any charges or prevent the service(s) from being moved. A copy of Photo ID is also required for account set up.
- You may qualify for a Lifeline discount. Lifeline is a government benefit program designed to reduce the cost of voice telephone service. Only
eligible consumers may enroll in the program and proof of eligibility documentation is necessary for enrollment. By law, Lifeline service is only available for
one phone per household, whether wireline or wireless service, but not both. Lifeline service may not be transferred, even to another eligible customer.
Consumers who willfully make false statements in order to obtain lifeline service can be punished by fine or imprisonment or can be barred from the

Information Required to Submit a Line Separation Request
(1) State that the survivor is requesting relief from the covered provider under section 345 of the Communications Act and our rules.
(2) Identify each line that should be separated using the phone number associated with the line;
(3) Regardless of which lines will be separated, identify which line(s) belong to the survivor and state that the survivor is the user of those lines.
(4) When a survivor is seeking separation of the line(s) of any individual under the care of the survivor, include an affidavit setting forth that any such individual is in the care of the survivor and is the user of the specific line.
(5) When a survivor is seeking separation of the abuser’s line, state that the abuser is the user of that specific line.

(6) Include documentation that verifies that an individual who uses a line under the shared mobile service contract (i.e., an “abuser”) has committed or allegedly committed a covered act against the survivor or an individual in the survivor’s care.
To meet the requirement for demonstrating survivor status, survivors must submit one or more of the eligibility documents prescribed by the SCA:
(1) A copy of a signed affidavit from a licensed medical or mental health care provider, licensed military medical or mental health care provider, licensed social worker, victim services provider, licensed military victim services provider, or an employee of a court, acting within the scope of that person’s employment.
OR
(2) A copy of a police report, statements provided by police, including military or Tribal police, to magistrates or judges, charging documents, protective or restraining orders, military protective orders, or any other official record that documents the covered act. The documentation provided should clearly indicate a known name for the abuser and the survivor, as well as include some kind of affirmative statement that constitutes an indication that the abuser actually or allegedly committed an act that qualifies as a covered act against the survivor or an individual in the care of a survivor.
